SUMMARY
Essential Behavioral Health is seeking a full-time Case Manager, CADC/Certified with CCAPP. The Case Manager is responsible for providing direct services and supportive contacts to the client and/or their families, and developing resources within the community, which are supportive to the recovery of the client. The Case Manager also functions as an advocate for the client, seeing that appropriate services are provided and assisting the client in receiving these services as appropriate. The Case Manager provides group therapy and related documentation services, along with additional ancillary services. The Case Manager is a highly qualified individual with experience regarding the nature of alcoholism and drug addiction. The Case Manager will need to be familiar with 12-Step Recovery programs and have knowledge of documenting group notes in an EMR system. Must have the ability to work as a team member and work collaboratively with the Primary Counselor and Clinical Director in providing quality treatment services and client care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered
2. Handle case assignments, draft service plans, review case progress and determine case closure
3. Communicate patient needs to a variety of care team members and follow up accordingly.
4. Manage discharge plans upon completion of treatment.
5. Attends clinical and administrative meetings for inter-departmental sharing of information.
6. Work collaboratively with patients, families, therapists, and referral sources to ensure high-quality care
7. Facilitate multiple care aspects (case coordination, information sharing, etc.)
8. Maintain the patient's comprehensive clinical record through detailed documentation
9. Help patients make informed decisions by acting as their advocate regarding their clinical status and
treatment options
10. Promote quality and cost-effective interventions and outcomes
11. Record cases information, complete accurately all necessary forms, and produce statistical reports
12. Helps navigate legal issues that patients are currently facing up to and including appearances in court
hearings.
13. Adhere to professional standards as outlined by protocols, rules, and regulations
14. Act as the patient's advocate as it relates to insurance coverage and financial assistance
15. Continually monitor and evaluate each client's progression through their case management plan, and
develop corrective action revisions to the plan as needed
16. Arrange abeyance with social services, health, and governmental agencies
17. Performs other duties as assigned by the supervisor, which is consistent with the position and in
compliance with agency policies and procedures.
EDUCATION / EXPERIENCE
* CCAPP (or equivalent certification body) certified CADC or higher.
* 1 to 2 years of experience as a group facilitator in Substance Abuse and Mental Health settings
* Bachelor's Degree in the human services field
* Minimum 1 year of full-time employment working in human services or in behavioral health sciences
* Knowledgeable of Department of Children and Families (DCF) guidelines and Joint Commission standards
* Must be familiar with 12 Step Recovery programs
* Knowledge of Kipu - preferred
